Vaccination Requirements
All students must be vaccinated for Measles, Mumps, Rubella (MMR). Students are encouraged to be vaccinated for Meningitis (MenACWY) and COVID-19.
You can take a picture or scan your records to upload as one file. If you have multiple pages, you can scan your vaccination record as a PDF on your iPhone or on your android. You can can also merge them into one PDF by using Adobe for free online.

Requirements for all vaccination records:
- Your form must show your first and last name printed (not handwritten).
- Your form must show your date of birth.
- You may take a picture of your form, but the photo must be clearly legible, and the above information must be present.
- If your record does not meet the above requirements you can download your vaccination record online at https://myvaccinerecord.cityofnewyork.us/myrecord/
- If you cannot locate your digital record, you may have a doctor fill out a new Immunization Form
Requirements for Measles, Mumps Rubella (required for everyone)
- Your form must show show dates of two shots for MMR administered 28 days apart and after 12 months of age
- Or you may show two shots for measles, one for mumps, and one for rubella
- Or you may show proof of immunity via a positive blood test for a previous infection.
Requirements for Meningitis (optional submission)
- If you are not vaccinated for Meningitis you will be asked to log into the Columbia patient portal and confirm your decision to decline the vaccination. If you are vaccinated you may submit your records.
- The vaccination must be from the last 5 years

- Trumenba (Meningococcal Group B)
- MENINGITEC (Meningococcal Serogroup C)
